Event Piercing Waiver

Please fill out the following form with the information of the person receiving the piercing(s)

By submitting this form, I agree to the following:

1. I am voluntarily requesting that The Luxe Beauty Lounge LLC perform body piercing(s)

2. I understand that body piercing is an invasive procedure that involves risks, including but not limited to: Pain, swelling, and bruising, bleeding, infection, allergic reactions, scarring, & nerve damage.

3. I have been provided with, read, and understand the aftercare instructions. I agree to follow these instructions to promote proper healing and reduce the risk of complications.

4. I understand that the success of the piercing and healing process depends largely on proper care and attention to the aftercare instructions.

5. I agree to contact a doctor or dentist with any issues with the piercing process.

6. I understand that body piercing is a form of body modification, and the final appearance may vary based on individual factors.

7. I confirm that all the information I have provided is accurate and complete to the best of my knowledge.

8. Release of Liability: To the fullest extent permitted by law, I hereby release, waive, discharge, and covenant not to sue The Luxe Beauty Lounge LLC, its owner,employees, agents, representatives, successors, and assigns (collectively referred to as "Releasees") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or the above mentioned minor, or to any property belonging to me or the above mentioned minor, while participating in the body piercing process, or while in, on or upon the premises where the body piercing is being conducted, regardless of whether such loss is caused by the negligence of the Releasees, or otherwise and regardless of whether such liability arises in tort, contract, strict liability, or otherwise, to the fullest extent allowed by law.

9. Indemnification: I agree to indemnify and hold harmless the Releasees from any loss, liability, damage, or costs, including court costs and attorneys' fees that Releasees may incur due to my participation in the body piercing process, whether caused by negligence of Releasees or otherwise.

10. I understand that this waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. Any portion of this document deemed unlawful or unenforceable is severable and shall be stricken without any effect on the enforceability of the remaining provisions.

11. I understand that this waiver does not affect my rights which cannot be waived under applicable state and federal law.

12. I consent to the use of appropriate piercing techniques as deemed necessary by the piercer.

13. I am aware that if I have any questions or concerns, I should consult with my healthcare provider before proceeding with the piercing.

14. I acknowledge that I have carefully read this waiver and release and fully understand that it is a release of liability. I expressly agree to release and discharge The Luxe Beauty Lounge LLC and all of its affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, from any and all claims or causes of action and I agree to voluntarily give up or waive any right that I otherwise have to bring a legal action against The Luxe Beauty Lounge LLC for personal injury or property damage.

15. Jewelry Change Policy: I understand and agree that if the initial jewelry needs to be changed for any reason (including but not limited to size adjustments, style preferences, or addressing complications), I am required to purchase the replacement jewelry directly from The Luxe Beauty Lounge LLC. I acknowledge that only jewelry purchased from The Luxe Beauty Lounge LLC will be used by their staff for any jewelry changes or adjustments to my piercing. I understand that this policy is in place to ensure the quality and safety of the jewelry used in my piercing, and to maintain the integrity of the piercing process.

16. I acknowledge that if I choose to use jewelry not purchased from The Luxe Beauty Lounge LLC, the staff will not be able to assist me with changing the jewelry.

17. I agree that any attempt to have The Luxe Beauty Lounge LLC staff change jewelry not purchased from their establishment may be refused, and that this refusal is within the rights of The Luxe Beauty Lounge LLC as outlined in this agreement.

By submitting this form & my signature below, I certify that I have read, understood, and agree to all the statements in this form, including the Jewelry Change Policy, and that all information provided is true and accurate. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.

As the parent or legal guardian of the minor named above, I hereby grant permission and consent for the above mentioned minor to receive the above mentioned body piercings from The Luxe Beauty Lounge LLC

1. I affirm that I am the parent or legal guardian of the previously named minor

2. I understand that all the risks, waivers, and releases described in the above sections apply equally to the minor for whom I am providing consent.

3. I acknowledge that I have read and understood all parts of this waiver and consent form.

4. I agree to indemnify and hold harmless The Luxe Beauty Lounge LLC from any loss, liability, damage, or costs that may incur due to the minor's participation in the body piercing process, whether caused by the negligence of The Luxe Beauty Lounge LLC or otherwise.

5. I understand that certain piercings may be prohibited for minors under state or local law, and I affirm that the requested piercings is legally permissible for a minor.

6. I agree to be present during the entire piercing procedure.

7. I understand that proper identification showing my relationship to the minor may be required.
